I, Guardian from Registration, legal guardian of Athlete from Registration,  a minor athlete, give express written permission, and grant an exception to the Minor Athlete Abuse Prevention Policy for 4591 Sports Development, an unrelated Applicable Adult to provide local vehicle transportation to Athlete from Registration (minor athlete)  to  and from Practice & 4591 Events for the duration of Oct 2019- April 2020

and further acknowledge that this written permission is valid only for the transportation during this time
